Eligibility

Participation Requirements :

To participate in any junior or high school sport, students must have a current Emergency Medical Authorization Form (EMF) filled out prior to conditioning, practice or try-outs and a current physical/ OHSAA Authorization Form (within the last calendar year) prior to practice or sporting event.  These forms may be picked up in the main office at Lakota Plains or downloaded from the Plains Athletic Website.

All students should submit their Physicals, OHSAA Authorization Form and participation fees to the Athletic Secretary in the main office.  Emergency Medical Forms will be required for each Sport and go directly to the Coaches of that Sport.

Upon completion of all necessary forms and participation fee the student will be eligible to participate.

STUDENTS WILL NOT BE ALLOWED TO PARTICIPATE WITHOUT A CURRENT PHYSICAL AND EMF…IT’S STATE LAW!

Academic Eligibility:

Student Athletes at Plains are required to meet Ohio High School Athletic Association, Greater Miami Conference, and Lakota Board eligibility requirements.

An athlete will be required to maintain a 1.0 Grade Point Average or pass 75% of their entire course load. Each week the athletes will be checked by their classroom teachers to see if they are in any academic trouble.

If an athlete is failing two classes, they will be ineligible to participate in their sport for that week. Parents will be notified by mail. If a student is in danger of being ineligible, students will receive notification at practice to take home.

If an athlete finishes a quarter with two failing grades, they will be ineligible for participation in any sport for the following quarter.
