FinalForms Now Available for Families

FinalForms Now Available for Families

As communicated with families throughout the summer, the district has moved to FinalForms for back-to-school form completion.

FinalForms is Now Live

New this school year, parents and guardians will be asked to complete school forms online through FinalForms. FinalForms can be accessed through OneLogin.

FinalForms will also allow you to update contact information anytime, automatically updating our school systems daily. Please note contact modification is no longer available in Home Access Center.

FinalForms Support

FinalForms offers support directly to our parents and guardians. Simply select the “Get Help” button at the bottom of any page.

FinalForms Translation

FinalForms offers translation to over 100 languages through their partnership with Google Translate. Simply choose your language of choice from the “Select Language” drop down menu at the bottom of any page.

OneLogin Reminder

OneLogin is our one-stop solution for students and families to access logins and links to Lakota’s online learning tools, such as FinalForms, Home Access Center (HAC), Canvas, SeeSaw (K-2) and other district parent guardian applications. 

The district will continue to phase out older login options, so parents are asked to please use OneLogin as the starting point to access any district online tools. 

Please note: Home Access Center (HAC) logins have been moved exclusively to OneLogin this summer. You will no longer be able to login to the traditional HAC login page. The eSchoolPLUS Family app will now also require you to sign on through OneLogin. 

Trouble Logging Into OneLogin?

  • Can’t access OneLogin: Please confirm your Lakota username format as described on the OneLogin page. Your username is usually your first name and last name with a period between, followed by For example:
  • Don’t remember your password: Please click “Forgot Password”, enter your Lakota username and follow the steps. A reset email will be sent to your personal email address on file with Lakota, including steps to reset your password. If you do not receive an email, please check your SPAM or Junk email folders.
  • Don’t know the personal email address on record at Lakota: Please check any personal email address you may have provided the district in the past. For additional assistance, please contact your student's school office for a password reset.
