Owner/Lease Holder Residence Affidavit

This form must be completed by the property owner or leaseholder.

As proof of residency, the owner must provide both of the following documents:

1. Current mortgage statement, Auditor's Report or Deed identifying them as the owner of the property.
2. Current monthly utility bill (electric, gas or water) - dated within 30 days of application date addressed to the owner.

As proof of residency, the leaseholder must provide both of the following documents:

1. Current rental agreement, signed by both Landlord and Tenant identifying them as the renter of the property.
2. Current monthly utility bill (electric, gas or water) - dated within 30 days of application date addressed to the owner.

All documentation must be submitted to the Lakota Welcome Center via:
