Parent-Guardian Residence Affidavit

This form must be completed by the parent or guardian.

As proof of residency, the parent/legal guardian must provide one of the following documents:

1. Current monthly utility bill (electric, gas or water), dated within 30 days of application date;
2. Financial statement or valid and current Government document addressed to the parent/legal guardian.

All documentation must be submitted to the Lakota Welcome Center via:

1. Upload documents to the student's FinalForms account through OneLogin.
2. Email: enroll@lakotaonline.com
3. In Person: Lakota Local Schools Central Office, 5572 Princeton Road, Liberty Township

This form is valid for the current school year only.

In order to ensure continued enrollment in Lakota Local Schools, you must complete a new Residence Affidavit from by the first day of school each school year.

• I agree that Lakota Local School District, if they deem it necessary, has the right to investigate my residency, including, but not limited to, conducting unscheduled home visits, to verify that I am living at the address listed above.
• I agree to allow the release of rental information to a representative of the Lakota Local School District. If it is determined that I am not a resident of the Lakota Local School District, I understand that my child(ren) will be withdrawn from the Lakota Local School District.
• I acknowledge and understand that falsification of the above information regarding legal residency and subsequent enrollment in the Lakota Local School District is a theft of service. If it is determined that the above information is falsified, I become responsible for tuition to the Lakota Local School District and subject to criminal or civil charges which may be filed by the Lakota School District (ORC Section 2921.13).
• If I move out of the Lakota Local School District, I will immediately notify Lakota local Schools at 513-682-4120 or enroll@lakotaonline.com and I will withdral my student(s).

For parents of athletes: I understand that Lakota Local Schools athletic teams will be forced to forfeit games when ineligible players who have enrolled under false pretenses are participating on the team.
