Required Documents

You will be asked to provide an original birth certificate (with official raised seal).

Parents or guardians enrolling students at all grade levels must provide their immunization records at the time of their scheduled appointment in order to complete enrollment.

If Renting or Owning
One of the following is required:

• Current deed
• Current mortgage statement
• Current Butler County property tax billing statement
• Settlement statement (within 90 days of purchase/closing)
• Lease or rental agreement
  The lease must be current and must indicate the term of the lease, address of the lease property, the name of the property owner, the name of the lessee and the listing of tenants. Also, must be signed by both lessee and property owner.

If Residing with a Lakota Resident
All of the following is required:

• The home/lease holder must provide their current lease agreement or mortgage statement, and photo ID.
• The enrolling parent/guardian must provide a current piece of official mail (pay stub, medical bill, or government document) addressed to the enrolling parent/guardian showing property address.
• The Residence Affidavit 1 and 2 (English) / Residence Affidavit 1 and 2 (Spanish) must be completed by both the home/lease owner and the enrolling parent.

The parent or guardian registering the child will be required to present a driver's license or state-issued ID card.

If enrolling a first grade student who did not attend a public or chartered non-public school for kindergarten, the Ohio Department of Education & Workforce requires that the parents/guardians provide confirmation from their kindergarten provider that the teacher met the certification requirements outlined by the Ohio Department of Education. 

The required documentation is the kindergarten teacher’s state teaching certificate or license. Families are responsible for obtaining the required documentation. Enrollment into first grade may be delayed until confirmation is received.

If applicable, please be prepared to provide any of the following: 

• Custody documents - Must be signed and court stamped
• Individualized Education Plan (IEP)
