Ohio law requires children to have certain immunizations to attend school. View the current list of required shots.
New Students: To complete the enrollment process, all students new to Lakota are required to provide their most recent record of immunization at the time of registration. We realize that summer birthdays may require more vaccinations, but the updated record can be submitted to the school nurse once school begins. Ohio Revised Code and Lakota board policy state that any child who has not met enrollment requirements must be excluded from school.
Returning Students: All updated immunization records must be provided to your school nurse before the 14th day of school (Sept. 5, 2025) to ensure your child can continue attending school. Records can be provided in a paper format or emailed/faxed from you or your doctor's office. Visit our Health Services page for a directory of Lakota nurses by assigned school.
Required immunizations, by grade level, are as follows:
Incoming Kindergartners: All students entering kindergarten are required to have:
- Four or more doses of DTaP or DT vaccine, or any combination. If all four doses were given before the fourth birthday, a fifth dose is required. If the fourth dose was administered at least six months after the third dose, and on or after the fourth birthday, a fifth dose is not required.
- Three doses of hepatitis B vaccine. The second dose must be administered at least 28 days after the first dose. The third dose must be given at least 16 weeks after the first dose and at least eight weeks after the second dose. The last dose in the series (third or fourth dose) must not be administered before age 24 weeks.
- Two doses of MMR vaccine. The first dose must be administered on or after the first birthday. The second dose must be administered at least 28 days after the first dose.
- Three or more doses of IPV vaccine. The FINAL dose must be administered on or after the fourth birthday with at least six months between the final and previous dose, regardless of the number of previous doses. If any combination of IPV and OPV was received, four doses of either vaccine are required. Only trivalent OPV (tOPV) counts toward the U.S. vaccination requirements. Doses of OPV administered before April 1, 2016, should be counted (unless specifically noted as administered during a campaign). Doses of OPV administered on or after April 1, 2016, should not be counted.
- Two doses of varicella vaccine. The first dose must be administered on or after the first birthday. The second dose should be administered at least three months after the first dose; however, if the second dose is administered at least 28 days after the first dose, it is considered valid.
Incoming 7th Graders: All incoming seventh-graders are required by law to have one dose each of the MCV4 (Meningococcal) vaccine and Tdap (Diptheria, Tetanus & Pertusis) booster vaccine.
Incoming 12th Graders: All incoming seniors are required by law to provide an updated shot record showing proof of two doses of the MCV4 (Meningococal) vaccine. If the first dose was given on or after the 16th birthday, a second dose is not required. A record of these updated vaccines must be sent to your school’s main office before the start of the new school year.
COVID-19 Vaccination: Lakota Local Schools does not require staff or students to receive the COVID-19 vaccine. If your student has received the vaccine, you can submit a copy of the vaccination record to your school's nurse to have on file.
Note: Parents/Guardians have the right to opt your child out the required vaccines for medical, religious or philosophical reasons. If you would like to waive specific immunization requirements, you must submit a signed statement by you and/or your doctor stating which vaccines you are waiving and the reasons why. Signed waivers must also be on file by 9/5/2025. Please contact your building nurse for more details.
