♦  Permission Slip Signed by Parent/Guardian Required for Each All Star Participating.

♦  Each Participating Student Will Be Allowed One Box Unless Parent/Guardian Comes to the Office and Pays for Additional Boxes In Advance.

♦  One Additional Box Will Be Sent Home Only If the Payment Envelope Is Signed By the Parent/Guardian, Authorizing Additional Chocolate.

♦  Parents/Guardians Are Responsible for Paying In Full for ALL Boxes They Request/Authorize to Be Sent Home With Their Students

♦  Cost is $60 Per Box - All Product Must Be Paid In Full, Whether All Sold or Not. 

♦  ** If your All Star participated last year, all past due funds must be paid in full - in advance - to be eligible to participate. **
